A Randomized Controlled Multicenter Trial of an Investigational Liquid Nutritional Formula in Women with Cyclic Breast Pain Associated with Fibrocystic Breast Changes
Based on the physical examination of the patient in particular a 38-year-old woman named Elaine Goodwin, except her medical history including IBS, migraines, exercise-induced asthma in addition to first degree of cystocele, this patient has fibrocystic breast disease. It could be the main diagnosis but a more in depth assessment along with practitioner asking the patient additional questions and recommend some tests to exclude such differential diagnoses as fibro adenoma, intra-cystic papilloma, chest wall pain, costochondritis, or breast cancer. In general, fibrocystic breast changes do not require specific treatment because this disease is rather common disorder and nearly 30-60% of all females experience it (Malherbe, 2020). It is described as a breast tissue thickening that can be palpated through the skin, and it can be without formation of any cysts or with cysts (Breastcancer.org, 2018). Thus, for confirmation of this diagnosis the doctor should ask the patient some questions. For example, the physician should know if the breasts are bothering Elaine and if the patient has specific areas of worsening or continuous breast pain. Moreover, the patient should answer the question if these changes in breasts persist after her periods. Additionally, the doctor should ask about possible family history of breast cancer. Although many various tests needed to exam breasts exist such as an ultra sound, a mammogram, and a breast biopsy, the doctor should offer to do this patient only an ultra sound. It is caused by the fact that a mammogram is not recommended for women younger than forty years old because it is x-rays of the breast (American Cancer Society, 2020). A biopsy is recommended only in case other tests show that the patient may have breast cancer. However, not each biopsy points to the cancer presence, but it is the only way to confirm or rule out this diagnosis.
